Whistleblower Submission

  1. Purpose

    Indigo encourages all individuals to come forward with any concerns regarding misconduct or wrongdoing within the organisation. This form is intended to assist you in submitting a whistleblower disclosure. Please be aware that you may be asked to provide further assistance during any subsequent investigation, if necessary. For more details, please consult Indigo’s Whistleblower Policy, which is available on our website.

  2. Are you an Eligible Discloser?

    Under the Aged Care Act 2024 anyone can make a Whistleblower Disclosure, if it is reported to an Eligible Recipient, please see Indigo’s Whistleblower Policy

    We’ll consider your information to determine whether you are eligible for protection under the Aged Care Act; National Disability Scheme Act 2013; and/or Corporations Act 2001. For more information about who is eligible for protection as a whistleblower, please see our Whistleblower Policy.

  3. Reporters Contact Information

    You may choose to make your disclosure anonymously, and your identity will be protected.
    If you wish to remain anonymous, this form can be posted to us via the address in our Whistleblower Policy.
    On submission, this report form is securely encrypted and is only accessible by the CEO and Board Chair.

  4. What happens after you make an eligible disclosure with Indigo?

    If you have given us your contact details, we will contact you to acknowledge receipt, and discuss the matter within 3 days, any further information we may need, and any steps we may take to investigate it.  

    Indigo requires that a protected disclosure is dealt with within 60 days from the date it is received. You can generally expect that the matter will be completed in this period. We will advise you if the investigation is likely to take longer.

 


You can remain anonymous, but this may limit any investigation of the disclosure, if we are unable to contact you to obtain more information as we progress our investigation. We will also not be able to notify you of the outcome of the investigation.

Alternatively, you could provide us with your name and contact details and ask that we don’t reveal them to others. This will help us investigate your concerns, and also protect your identity.

We are open Monday to Friday, from 8:30am - 4:00pm.
