How to Access Indigo's Aged Care Services
Understanding how to access the support you need is an important first step in living an independent and comfortable life in your own home.
The below information is suitable for users who are funding their care through the Commonwealth Home Support Programme or Support at Home schemes. If you are funding your care through an alternate scheme or self-funding, please Contact Us to find out the most appropriate steps for you
Step 1: Organise an assessment with My Aged Care
An assessment with a My Aged Care assessor is the first step to accessing aged care services with any provider. Assessments are carried out in your home by a trained assessor with the goal of better understanding what aged care support is right for you and what funding options may be appropriate for your needs
Step 2: Receive your assessment outcome
Once you’ve been assessed, you’ll receive a Notice of Decision letter that will outline your assessment outcome. This letter will tell you if you are eligible for government-subsidised services, which program you will be supported under and your support plan (if eligible).
Your support plan will also include information you need to access the services you may need. This can include a referral code you can share with your chosen service provider so they can start organising the help you need.
Step 3: Organise an appointment with an Indigo Therapist
Once you’ve been approved for funding, an appointment with your Indigo therapist will focus on how we can support your health, movement and safety. Our team visit you in your own home and will watch how you move and what tasks you may struggle with. This helps us set goals like:
- Walking safer,
- Getting stronger, and
- Doing day-to-day tasks easier.
If you were approved for the Commonwealth Home Support Program (CHSP)
You are able to access Indigo’s services information either via your My Aged Care Online Account or by contacting the provider directly. Let us know what your referral code(s) is and we can use this to view your client records and support plan details.
Our team will then work with you to find availability of an Indigo therapist in your area.
If you were approved for the Support at Home program (SAH)
Indigo is happy to work alongside as an associated provider for Allied Health services alongside your registered Support at Home provider.
If you were receiving services with Indigo as a part of your previous funding and wish to continue, please request us as an associated provider with your primary provider. We are happy to provide them with previous assessments and results based on working towards your goals.
If Support at Home is your first funding program, discuss with your primary provider some of your goals for independence at home and how Indigo’s allied health providers can help you thrive.
Step 4: Ongoing Support
If you have previously received support from Indigo but find your needs have changed, you are able to request a support plan review at any time through the My Aged Care Portal. Indigo is able to help with a range of physiotherapy or occupational therapy services that may help you live independently at home and in the community.
