The Role
- Play a pivotal role in improving clients' independence, wellbeing, and quality of life.
- Perform assessments within clients' homes to understand their needs.
- Deliver clinical interventions, including recommending assistive technology and home modifications.
- Provide expert advice, assessment, and prescription of assistive technology and home modifications for CHSP eligible clients.
- Assist with coordinating a case load, including planning, service delivery, and evaluation.
- Ensure accurate and up-to-date clinical documentation.
- Identify and complete referrals to other services as needed.
- Work in a hybrid role, involving in-home visits, office work, and remote work from home.
- Primarily based in the Metro North area, with consideration for other metro regions.
The Candidate
We are seeking a passionate, self-driven, and innovative Occupational Therapist to join our team. The ideal candidate will be highly motivated, demonstrating a strong commitment to enhancing client independence and wellbeing. You'll be organised and flexible, with excellent communication, problem-solving, and initiative, and comfortable working autonomously within a community setting. Sound clinical expertise is essential, and experience within the Aged Care sector is preferred.
The right candidate will have:
- Passionate about delivering compassionate, community-based aged care that promotes dignity and independence.
- Skilled in collaborating with clients, caregivers, and healthcare professionals to achieve positive client outcomes.
- Fully certified with AHPRA registration, NDIS Workers Screening, WA Driver’s License, and current First Aid & CPR.
- ICT skills (including use of databases and Microsoft Office suite applications).
- Experience working with people with disabilities and/or experience with Assistive Technologies.
