Entry Form

Compulsory components of your submission include:
• A completed entry form (each entry you make must be treated individually with its own entry form and fee)
• Poster artwork submitted by email in the requested format
• A 50 word description of your poster and a 100 word biography
• You are also welcome to include (but it is not compulsory) the words: MIX08 and Indigo on the poster

All Fields must be completed to send valid entry form.
